(904) 880-9001 (V/TTY)DIRECTIONS BY CAR FROM EITHER NORTH OR SOUTH OF JACKSONVILLE:Take Interstate 95, from north to south, to interstate 295, which is approximately 9 miles south of the Jacksonville center. Take I-295 North (Toward Orange Park) to the first exit, No. 3, (approximately 1 mile) which is St. Augustine Road. At the Exit Ramp turn to the right and go north on St. Augustine Road. Continue on St. Augustine for approximately 2 miles, passing through four traffic lights. Clarke Jacksonville is located on the right just past Crown Point Road. DIRECTIONS BY CAR FROM WESTERN FLORIDATake Interstate 10 East until it terminates into Interstate 95. Take I-95 South approximately 9 miles to Interstate 295. Take I-295 north and follow the instructions above. We are located in the two-story, white, stucco building (The Doctors Center) on the second floor (suite 6 at the top of the stairs).
